Care at home

A physical therapist who comes to you, for people for whom a trip to the clinic is hard, or who want to practice in the place a fall worries them.

Exercise balls, small dumbbells and a resistance band on a rug in a living room

Is this for you?

This care is for you if

  • Getting to the clinic is hard for you or your driver

  • You worry about a certain room, step or doorway at home

  • You are home from the hospital and are not ready for a trip out

  • You would like your family to see the exercises where you live

A visit

What a visit looks like

The first visit takes about an hour. Later visits are shorter. One of our four physical therapists treats you, and they stay beside you.

  1. 01

Step 1: We agree on a time window

The front desk gives you a window for the visit and calls if we are running late.

  2. 02

Step 2: We look at the rooms you use

Floors, rugs, steps, chairs and lighting, talked through with you and your family.

  3. 03

Step 3: We practice where you live

Standing, walking and stepping on your own floors and stairs, with a therapist beside you.

  4. 04

Step 4: We leave a plan

Exercises in large print and a plan for the next visit.

What we do

How we work with you

  • A home walk-through

What is easy, what is awkward and what could be changed.

  • Balance and strength practice

The same care as at the clinic, with your own furniture and steps.

  • Stairs and thresholds

Practice on the steps you use every day.

  • What the therapist brings

A gait belt, a balance pad, bands and light weights, carried in a bag.

  • Family training

The person who helps you learns what to do, in the room where they will do it.

  • A path back to the clinic

When you are ready, we can move your visits to the clinic.

Where we go

The towns we visit

Altagracia and the other therapists travel to these towns around Scranton.

Lackawanna County

  • Scranton
  • Dunmore
  • Throop
  • Olyphant
  • Dickson City
  • Taylor
  • Old Forge
  • Moosic

If you live outside these towns, call us and we will tell you whether we can come.

What decides how many visits

How many visits you need depends on what we find, what you want to be doing, how you respond and what your plan covers. We go over the plan with you as we go.

Who treats you

A physical therapist plans and leads your care.

  • Physical therapist in a light grey polo in a sunlit clinic hallway

    Thuy Hoang

    Physical therapist and clinic lead

    • Physical Therapist · PA Lic. 000000

Thuy leads the clinic and meets many people at their first visit. She likes to start with what a person wants to be doing again, and she asks the family to join in.

  • Physical therapist in a black athletic top in a sunlit clinic hallway

    Pauline Ashford

    Physical therapist

    • Physical Therapist · PA Lic. 000000

Pauline works on strength and walking, and on getting up from a chair or a car with less effort. She writes every home exercise in large print.

  • Physical therapist in glasses and a navy polo beside the parallel bars

    Minoru Sato

    Physical therapist

    • Physical Therapist · PA Lic. 000000

Minoru sees people after a joint replacement and people who live with Parkinson's. He works closely with their surgeons and neurologists when they ask him to.

  • Physical therapist in a navy polo resting a hand on the parallel bars in the clinic gym

    Altagracia Palacios

    Physical therapist

    • Physical Therapist · PA Lic. 000000

Altagracia makes the home visits. She looks at the rooms and steps a person uses every day, and she shows the family how to help.

How it is paid for

Medicare Part B can cover outpatient PT at home if you are not getting home health at the same time. If a home health agency is already visiting you, tell us and we will explain what changes. Many other plans cover home visits too, and the front desk checks yours before the first one.

When to see a doctor first

Call your doctor about a fall that left you hurt or new dizziness, and call 911 in an emergency. If you are not sure whether a home visit is right for you, call the front desk and ask.

Questions about this care

Do you charge for travel?

The front desk explains how home visits are billed before you book.

Can you see me in two rooms?

Yes. We look at every room you use, and we practice where it is hardest.

What if I have home health already?

Tell us. The rules for Medicare change when a home health agency is involved, and we explain what that means for you.
