Sending someone to us

We see adults 65 and over, at the clinic and at home. Here is how to send a referral and what comes back to you.
The route
From your office to our door

Step 1: Send the referral
Use our secure referral form, or call the front desk and we will send you the fax details.

Step 2: We call the person
We contact the person within the working day and set a first visit, at the clinic or at home.

Step 3: We check coverage
Our front desk checks Medicare and any other plan before the first visit.

Step 4: You hear back
You receive our evaluation note and a plan of care to review and sign when Medicare asks for one.
