For families and caregivers

Bringing someone to us? Start here.

Directions, parking and what to bring, all on one page, for the person who drives and the person who helps.

The clinic is at 1128 Mulberry Street in Scranton. Coming from Interstate 81, follow the signs for downtown Scranton and then Mulberry Street. Coming from downtown, follow Mulberry Street uphill. The building is a one-story clinic with a stone front, a glass door and a planter by the entrance.

If the day is a hard one, call us: the front desk can talk you through the last turn.

Parking

Where to park and where to drop off

  • The lot

Our lot is behind the building, off Mulberry Street. The spaces nearest the door are marked for patients.

  • Dropping someone off

Pull up to the door, and our front desk comes out with a chair. Then go and park.

  • From the car to the door

A level path leads to an automatic door, with no steps.

  • When it rains or snows

Tell the front desk. We bring a chair to the car, and we keep the path clear.

What to bring

A short list for the day

  • Insurance cards

Medicare, and any other plan or supplement card.

  • A photo ID

A driver's license or other photo ID.

  • Medicine list and doctor's notes

Whatever your doctor has given you, if you have it.

  • Glasses and hearing aids

    Whatever is used every day.

  • The walker or cane

The one used at home, so we can see it in use.

  • Flat, closed shoes

Sneakers or shoes with a back and a flat sole.

During the visit

Where you can be

  • In the gym

There is a seat for you beside the exercise area. Stay as close as you like.

  • In the waiting area

If your parent would rather work on their own, there is coffee and a quiet seat.

  • On the phone

If you cannot come, we can talk by phone, with their OK.

  • At the end

We go over the exercises with you, in large print, so you know what to expect at home.
